Safety Alert – Injuries – 02-10-2025

Brothers and Sisters,

Attached and below are three Incident Summaries involving two Incident-Injury-Struck By and a Hand & Power Tools, incidents. Please review.

Hazard:

1 – Incident-Injury-Struck By

A construction crew was working in the pit installing rail brackets and the first set of guide rails. Plumbers were working in the lobby area of the top landing and accidentally kicked a piece of pipe down the hoistway, striking the temporary mechanic in the face.

2 – Hand & Power Tools

An apprentice was sent to the machine room but had not been given keys to access the machine room door. The apprentice proceeded to use a pocketknife to pick the machine room door lock and in doing so, the knife slipped and cut his finger.

3 – Incident-Injury-Struck By

A repair crew was installing a hydraulic jack using a nylon strap wrapped multiple times around an overhead beam as a pick point, with a snatch block attached. The crew was hoisting the jack to place shim for final alignment when the strap broke. The snatch block fell from the overhead striking the mechanic on the head.
