Safety Alert – Injuries 01-06-2024

Brothers and Sisters,

Attached and below are three Incident Summaries involving an Incident-Injury-Other, and two Incident-Injury-Caught Between. Please review.

Hazzard:

1 – Incident-Injury-Other

A mechanic was lowering an escalator controller back into the well when the legs of the controller got caught on the truss. When he pulled on the controller to free it from the truss, he felt his back “pop”

2 – Incident-Injury-Caught Between

A repair crew removed a drive sheave for replacement and during the process the sheave’s outer circumference was fractured. While the apprentice was trying to roll the sheave to a secure location, he lost control, and the sheave fell towards him causing him to fall with the sheave landing on his legs

3 – Incident-Injury-Caught Between

A mechanic and an apprentice removed the guard from the car top sheave to inspect the sheave and check the tensioning of the CSBs. During their inspection, the mechanic instructed the apprentice to run the car in the up direction. The mechanic did not consider the placement of his hand and when the car ran up, his fingers were pulled in between the belts and the sheave
