Authorization for Release of Motor Vehicle Records from Department of Motor Vehicles

I, the person named and Identified below, as an employee of ThyssenKrupp Elevator Corporation, hereby authorize the Company or its fleet risk management firm to periodically obtain and release information to ThyssenKrupp Elevator Corporation pertaining to my driving record from the Department of Motor Vehicles of all states in which I have resided within the last three years, for the purpose of determining my eligibility to operate and/or continue to operate a company owned vehicle. Furthermore, should any State have its own designated forms that are required to obtain my driving record, I agree to complete such forms as required under the applicable state law. I agree that this authorization will become effective immediately and continue until I notify the Company in writing that I withdraw this authorization.

Following Clarification/Understanding Hereby Incorporated Into This Authorization:

1. When conducting/requesting an MVR check from a state agency, the time period covered by the check is established by each State. The company does not dictate the time period In this matter,

2. Each State establishes its own time frame for the period of time covered by the MVR check - the "average" time frame is five years, and the minimum period furnished is three years.

3. Irrespective of the time frame established by the State and the information furnished to the company, the company will only be reviewing/concerned about the immediately preceding three years of driving history.

4. The MVR Information will only be used by the company for purposes of determining the eligibility of employees to operate or continue to operate a company vehicle. The information in the MVR report is intended exclusively for use by the company and will not be released of made available to any other parties.

More specifically, an additional clarification has been requested on two portions of the first sentence, ".the Company or its fleet risk management firm to periodically obtain and release Information pertaining." The following will clarify these two Issues:

· "or its fleet risk management firm" - the Company has made arrangements to use the database services of a fleet risk management firm, for purposes of accessing the MVR information. The firm is a global fleet service firm delivering services to some of the largest fleets in the world. The MVR information will come from the firm's database, directly to the Company's Fleet Department, via a secure internet based platform.

· "and release"- this phrase refers to the Information being released from the fleet risk management firm's database to the Company NOT to any other party. As noted In Item 4 of my December 9, 2004 letter to the IUEC General President Dana Brigham, the Information in the MVR report is intended exclusively for use by the Company and wilt not be released or made available to any other parties.
